view the accompanying CAPA Memorandum
for Deans, Directors and Department Chairs.
The CAPA Application form is available for download in two formats:
- MS Word format, which can be
filled out and edited with any Word-compatible word processor
- Adobe Acrobat format, which can be filled out and edited
using the free
Adobe
Reader program.
You may either “left click” on the links to open the form
in your browser, or “right click” on them to save them
to your hard drive.
If you are having difficulties accessing these forms, you may also
contact Ms. Pam Solomos (x50358, or
psolomos@umresearch.umd.edu)
and have her send you hard copy of the form.
Please note that ann original application plus 9 copies on white paper
must be submitted to CAPA, The Graduate School, Room 2125 Lee
Building, by 12:00 p.m, Friday, 1 October 2004.
